As a UX designer, my brain is always looking for the design patterns hidden in pop culture. So when I started thinking about the potential of AI in healthcare — what I call the Encom Vision — my mind immediately went to the movie Tron: Ares and its central theme of using a digital Grid to solve human problems. But what if that digital world has a flaw? That brings me to the research I had done on vocal biomarkers: a technology with the power to detect conditions from Parkinson’s disease to depression with nothing more than a voice sample.
The potential is vast, addressing an urgent global need. As the scientific review Voice for Health: The Use of Vocal Biomarkers from Research to Clinical Practice confirms, this area of research is constantly expanding its use from detecting neurodegenerative diseases to monitoring other conditions in clinical settings.
But within this incredible promise lies a hidden, foundational weakness — a “Dillinger’s Flaw” that threatens the entire system. In AI-driven healthcare, this flaw is the “black-box” problem. The only way to bridge the gap between AI’s immense power and the trust it needs to be used safely is through responsible, human-centered User Experience (UX) design.
The philosophical challenge of AI in healthcare: transforming the opaque “Black Box” into a transparent “Glass Box” of accountable logic. UX design is the bridge.
AI bias is not a simple technical glitch; it is a fundamental failure of user-centric design — it is the digital embodiment of prejudice. This is the “Dillinger Problem,” a reference to Julian Dillinger’s self-serving flaws in the Tron universe.
This problem becomes life-critical in healthcare. Research published in the NIST Special Publication 1270 warns that AI bias stems from deeper layers of human and systemic biases embedded in our data. Vocal biomarker models are just as vulnerable to bias based on different accents, languages, genders, and ethnicities.

The solution is proactive design. The User Interface (UI) must be designed for transparency. By clearly disclosing “data provenance” — the origin, diversity and quality of the datasets — the system empowers clinicians to anticipate and mitigate potential biases. For patients, the prospect of having their voice recorded and analyzed by an AI raises immediate and valid fears about privacy. In this high-stakes context, frictionless design becomes a Dillinger’s Flaw in the trust architecture. A seamless, one-click ‘Accept All’ button is a hidden weakness because no real trust was ever built.
The best UX for building durable trust is a “trust-building design pattern” that introduces “intentional friction” into the consent process. This “Ethical Leap of Faith” — forcing the user to stop, consider, and actively grant specific permissions — builds a foundation of trust.
This process directly implements the ethical principles outlined in the WMA Statement on Artificial and Augmented Intelligence in Medical Care, which states that technology must “augment, not replace human judgment, preserving the physician’s central role in patient care.”
The ethical exchange: When designing for sensitive data consent, the goal is not to eliminate friction, but to make the choice intentional. This deliberate action (the push of a button) is the foundation of trust.
Physicians are understandably reluctant to trust recommendations from systems they don’t understand. The solution is to position the AI as a co-pilot that provides augmentation, not a replacement that brings automation.
The principle is that AI should “augment the capabilities of healthcare professionals.” Think of the physician as the user controlling the Light Cycle, and the XAI as a trustworthy program running on the Grid, providing only the necessary data to navigate the path. The Light Cycle isn’t driving itself. Critically, the final responsibility for diagnosis and therapy “must always lie with the physician”.
To systematically build trust and fulfill the Encom Vision responsibly, deployment must follow an iterative, human-governed approach:
Phase 1: Academic Testbed: Deploy first in academic centers to prove the model’s statistical validity.
Phase 2: University Hospital Pilot: Move to teaching hospitals for the first “in the wild” deployment, strictly governed by experienced clinicians.
Phase 3: Mass Diffusion: Only after proving the utility can the AI be mass-deployed, aligning with UN SDG Goal 3: Good Health and Well-being.

The path to integrating AI into healthcare safely and effectively is paved with deliberate, human-centered design choices. It requires three core principles of responsible UX for medical AI: fighting the Dillinger Problem by actively pursuing data diversity; using ethical friction to build patient trust; and positioning AI as an augmentative co-pilot for the clinician.
For healthcare leaders, designers, and data scientists, the evolution mandate is clear. Responsible UX Design is not an optional feature but the core operating system for trustworthy AI. We have a problem with an economic burden of nearly $52 billion every year in the US, according to a study published by the Michael J. Fox Foundation. The solution lies in our design decisions.
As we build these AI co-pilots and strive for the Encom Vision, we must remember the gravity of this digital creation:
